If you would like to register NWSRA programs on-line, use the On-Line EZ Registration System. If you have already signed up to use this system, click "Log In" above.

To register for NWSRA programs on-line, an On-Line Account needs to be created. Please call the NWSRA office to receive a form or download a form by clicking on the link below and print it. Once the form is completed, return the form to NWSRA by mail or fax to 847/392-2870.

Once the form is received by the NWSRA office, we will issue one Family Account Number and an Individual Participation Code for each member of the household who could be registering for NWSRA programs. Complete the information about each person or persons with disabilities. In addition, please include other family members if you anticipate their participation in an NWSRA program. For example, if your child with a disability and a sibling register for Sibling Soccer, you will need these additional numbers to complete the registration process on-line.

For security purposes NWSRA does not assign passwords. Once you are issued a login ID by the NWSRA office, follow the steps below to obtain your password and gain access to the EZ Registration System.

STEP 1: Click the "Log In" text at the top of this page. This will take you to EZ Registration System login page.

STEP 2: In the middle of the login screen page, click on the the link "Forgot your password?".

STEP 3: Enter your email address and click on the "Continue" button. Your new password will be emailed to you.

STEP 4: Obtain your new password from email, go back to the login page and Click on "Return to Log In". Enter your login ID and password to gain access to the system.
